Please read the above information and submit the  trip consent form below by 12:00 Noon, 10/15/2023.  If you do not wish your child to participate, your child must stay home the day of the trip. There will be no one at school to supervise your child.

Please submit the form even if you do not wish your child to participate. Please note that due to transportation booking, the due date to submit the form is 12:00 Noon, 10/15/2023.
